    Data collection

    The data for the GENE-SMART study (Genetic Evaluation for breast cancer in the Netherlands: Studying a Mainstreaming Approach for Redefining Testing pathways), a nationwide retrospective cohort study, was obtained from the Netherlands Cancer Registry (NCR), kept by the Netherlands Comprehensive Cancer Organization (IKNL). Specially trained registrars gathered data about patient, diagnostic, tumour, treatment characteristics and whether genetic testing was performed, directly from patient files in almost all Dutch hospitals (73 of 75 hospitals). Two hospitals were excluded from the study as data from these hospitals was not collected by data registrars employed by IKNL. In the remaining hospitals (n = 73), patients were excluded if they had undergone genetic testing before their breast cancer diagnosis or if data about the hospital of diagnosis, tumour stage or postcode was unknown.

    Study sample.

    The study included newly diagnosed Dutch breast cancer patients diagnosed between 1 January 2017 and 31 December 2022. Patients aged 18 or older were eligible for genetic testing if they met the patient characteristics as described in the Dutch guidelines [21].

    Variables

    Organization of genetic care

    Three pathways to access to genetic testing apply in the Netherlands: (1) referral to a genetic HCP in one of the eight hospitals with a genetics department (seven UMCS – university medical centres – and the Antoni van Leeuwenhoek cancer hospital), (2) referral to an academic genetic HCP at a satellite outpatient clinic embedded in a general hospital (which is still run by genetic HCP from a genetics department) or (3) the MGT pathway, which is the pre-test counselling by the physician who is treating the patient. Information from the genetics departments indicated that a period of six months was generally considered a reasonable timeframe for the full implementation of MGT. Therefore, the MGT pathway was subdivided in the MGT start-up phase (< 6 months after the start of the implementation of the MGT pathway) and fully-implemented MGT (≥ 6 months after the start of the implementation of MGT pathway).

    The organization of genetic care during the study period was determined for each Dutch hospital, based on information obtained from the eight genetics departments in the Netherlands. At the end of the study period, 26 out of 73 hospitals had implemented the MGT pathway, compared to 3 hospitals at the start. Patients diagnosed at a hospital where patients were referred to a genetics department (i.e. from a general hospital or from the hospital in which the genetics department was situated) were categorized as ‘referral to genetics department (RGD)’. Patients diagnosed at a hospital where patients were referred to a satellite outpatient clinic were categorized as ‘satellite outpatient clinic’. If MGT was the current practice in a hospital at the patient’s date of diagnosis, a comparison was made between the patient’s date of diagnosis and the date when MGT was started. Patients diagnosed less than 6 months after the start of the implementation of the MGT pathway were assigned to the ‘MGT start-up phase’ group and patients with a date of diagnosis more than 6 months after the start of the MGT pathway in their hospital were assigned to the ‘MGT’ group.

    Eligibility criteria for genetic testing

    Based on the Dutch national breast cancer guideline [21] patients eligible for genetic testing were divided into four groups: breast cancer diagnosis at age < 40, triple negative breast cancer (TNBC) at age < 60, bilateral breast cancer with the first diagnosis at age < 50, or male breast cancer (Table 1). These groups were not mutually exclusive. The four eligibility criteria were included in the logistic regression model as separate binary variables, allowing for overlap between criteria. Other genetic testing criteria (e.g. family history, Ashkenazi Jewish ancestry) exist, but are not included in this study, since this data was not available.

    Tumour stage

    Tumours were grouped according to the TNM staging system: Ductal Carcinoma In Situ (DCIS) and tumour stages I, II, III and IV. When the pathological stage was unknown, the clinical stage was used.

    SES

    Median household income was used as a proxy for socioeconomic status. Statistics Netherlands provided data about household incomes broken down by detailed postcodes for 2016, with each postcode covering an average of 17 households. Median household income was clustered into 9 groups, covering 99% of the Dutch population. SES scores were classed as ‘low’ (1–3), ‘middle’ (4–7) or ‘high’ (8–9), adapted from Van Maaren et al. [22]. Previously, SES was determined using a score derived from mean household income, the proportion of residents with low income, the percentage of individuals with limited educational attainment, and the unemployment rate [22]. However, this method is not used anymore since July 2023 due to the lack of data. The correlation between the prior methodology and the current approach is strong (r = 0.72, p < 0.0001).

    Orthodox religion

    The patient’s postcode was also used for determining whether a patient lived in a Reformed church municipality, as a proxy for orthodox religion. Based on a recent study [23] we created a list with the percentage of votes per municipality for the Staatkundig Gereformeerde Partij (SGP), the religious orthodox Christian political party, during the 2017 Dutch parliamentary elections [24]. Municipalities were grouped based on the percentage of votes for the SGP there: ‘low’ (less than 5%), ‘moderate’ (5 to 10%), ‘high’ (10 to 20%), and ‘very high’ (more than 20%). Although the proportion of votes for the SGP in the 2017 parliamentary elections were similar to those in the 2021 parliamentary elections, mergers of various municipalities in 2019 that led to more postcodes per municipality made the 2021 parliamentary election results less accurate. We therefore only used the results of the 2017 parliamentary elections.

    Travel time to genetic care

    Travel time to the nearest HCP for genetic care at the time of the patient’s diagnosis (i.e. to one of the eight clinical genetics departments, a satellite outpatient genetics clinic or an MGT hospital) was calculated using the Drive Time Matrix (Geodan, version 2.1), containing all travel times between Dutch postcode areas. Travel times were grouped into < 10, 10–20, 20–30 min and > 30 min.

    Statistical analysis

    Genetic testing uptake was determined based on the percentage of eligible patients who received germline genetic testing. Chi-squared tests were carried out to identify differences in genetic testing uptake based on the organization of genetic care, both for eligible patients overall and for specific clinical genetic testing criteria defined by patient characteristics. Multivariable logistic regression analysis was performed to determine the effect of MGT and SES on the uptake of genetic testing by eligible patients. For estimating the odds ratios (ORs) of MGT and SES for the uptake of genetic testing, the four clinical genetic testing criteria based on patient characteristics (each criterion included individually), tumour stage, year of diagnosis, orthodox religion and travel time to a genetic HCP were included as confounding variables. The same confounding variables were used for estimating the effect of the MGT on the uptake of genetic testing for patients within different SES groups. To determine the effect of MGT on the disparities in uptake of genetic testing between patients in different SES groups, ‘MGT’ was compared against ‘RGD’, ‘satellite outpatient clinic’ and ‘MGT start-up phase’.

    We assessed multicollinearity among predictor variables by calculating the Variance Inflation Factor (VIF). All VIF values were below 5 (mean VIF = 1.93), indicating no significant multicollinearity. The analyses were carried out using Stata 17.0. A p-value of < 0.05 was considered statistically significant.

    Nintendo Switch 2 owners feel like the console is missing some basic features from the PS5 and Xbox Series X. Nintendo has never really followed the pack when it comes to gaming. The gaming giant has always innovated on its own with things like motion controls, console/portable hybrids, and more. With that said, Nintendo has been criticized for also being outdated in some ways. The Nintendo Switch’s voice chat function wasn’t on the console, it was only usable through an app on a smart phone. There are no achievements on Nintendo’s hardware. These are just some of the things that show Nintendo is kind of behind the times when it comes to modern gaming.

    With that said, the Nintendo Switch 2 has also been criticized for being its least innovative console. It’s a more polished Switch 1, but that’s also part of its upside. The Switch 1 was a great console, but it needed some more refinement. The system generally feels more premium both from a hardware perspective, but also the software inside of it. It’s a smoother console that makes small but meaningful improvements from its predecessor. With that said, some are frustrated that the Switch 2 is missing features that its competitors have had for quite some time.

    Reddit user comfybonfire expressed frustration over the fact that the Nintendo Switch 2 doesn’t have a “Cart” feature on its eShop. On Xbox Series X and PS5, players can add a bunch of games to a digital cart and then check out all at once. It makes it super convenient to buy multiple games, particularly during a big sale. However, Nintendo doesn’t have that. You have to go buy each game one by one, which can be really annoying since the eShop is a bit clunky.

    “Genuinely I can’t think of a reason for why Nintendo didn’t add a Cart feature for the Switch 2 eShop,” said the Reddit user. “They added the ability to filter your wishlist by games that are on sale (which I love) but you still have to individually buy each game. For god’s sake there was a “Add to Cart” feature for the PS3’s digital store. Why couldn’t Nintendo add it for the Switch 1 or at least the Switch 2? At this point, it feels like a deliberate decision to not have a Cart feature, I just can’t think of a reason as to why.”

    Although they speculated this is “deliberate”, it’s not super uncommon for Nintendo to be behind the curve. The Switch 2 made the eShop much more enjoyable to use by making it faster and smoother to use, so maybe the lack of a cart was somehow a performance thing with the previous Switch. Some suggested this is a way to prevent users from second guessing their puchases. If it’s in a cart, you have time to think about it and can see the price racking up. If it’s not in a cart, impulse buys are more likely.

    It’s hard to say, but other Reddit users agreed with the post. Some noted that the enhanced speeds of the Switch 2 eShop made them want to buy more games, but the accessibility of doing so counters that to a degree. Maybe one day Nintendo will add this feature, but I can’t imagine it’s at the top of their priority list.

    LONDON (Reuters) -The world’s sovereign wealth funds are turning to active fund management and investments in China, while central banks are diversifying reserves to weather a volatile global environment, an Invesco survey of sovereign funds and central banks managing $27 trillion in assets showed.

    Still, the dollar reigns supreme, with the bulk of central banks saying it would take two decades to dethrone it – if ever – as the top reserve currency despite growing concerns.

    “Institutions with greater than $100 billion – so the pretty large institutions – those are the ones that were most interested in moving more to active management,” said Rod Ringrow, Invesco’s head of official institutions.

    Whereas funds liked passive management in predictable market conditions, predictable was “no longer the case,” he added. “I think that frames the whole approach… in this move to active management.”

    On average, wealth funds made returns of 9.4% last year, the joint second-best performance in the survey’s history.

    Nevertheless, market volatility and de-globalisation concerns have spiked – and over the 10-year horizon, big worries centre around climate change and rising sovereign debt levels.

    Over 70% of the 58 central banks polled for example now believe rising U.S. debt is negatively impacting the dollar’s long-term outlook.

    Nevertheless, 78% think it will take more than two decades for a credible alternative to the greenback to emerge. That is a jump from 58% last year while just 11% of central banks now view the euro as gaining ground compared to 20% last year.

    CHINA FOMO

    The survey was carried out between January and March – before U.S. President Donald Trump’s “Liberation Day” tariff announcements and at the peak of excitement around DeepSeek AI’s emergence in China.

    Wealth funds are seeing a major resurgence in interest in Chinese assets with nearly 60% intending to increase allocations there in the coming five years, specifically the tech sector.

    That number jumps to 73% in North America despite the worsening U.S.-Sino tensions, whereas in Europe it sits at just 13%.

    Wealth funds, the survey said, were now approaching China’s innovation-driven sectors with the “strategic urgency they once directed toward Silicon Valley.”

    “There’s a little bit of a FOMO,” Ringrow explained, a view that “I need to be in China now” as it shapes up to be a global leader in semiconductors, cloud computing, artificial intelligence, electric vehicles and renewable energy.

    The Emirates World Rugby Match Officials have been confirmed for The Rugby Championship 2025, which takes place between 16 August and 4 October.

    A total of 11 referees representing six nations, and boasting 331 tests worth of experience, will take charge of the 12 matches. The 2025 edition of The Rugby Championship will see four of the world’s top eight-ranked teams battle it out across four continents and 11 cities.

    Ben O’Keeffe (New Zealand) will take charge of the opening match between South Africa and Australia at Emirates Airline Park in Johannesburg on 16 August, with Andrea Piardi (Italy) to referee the last match between Argentina and South Africa at Allianz Stadium in Twickenham on 4 October.

    Porsche once again makes it onto the WEC podium in Brazil

    Porsche Penske Motorsport got off to a strong start in the endurance race at the Autódromo José Carlos Pace: in front of almost 85,000 enthusiastic fans, Frenchman Julien Andlauer was able to attack the Cadillac, which had started from pole position, on the second lap and overtake it shortly afterwards with a spirited manoeuvre. The number 5 Porsche 963 thus took the lead and built up a small advantage. At the first pit stop, the factory team changed the racing tyres on the right-hand side of the car only. This pattern was to be repeated at the halfway point of each double stint: as the 4.309-kilometre Sao Paulo Grand Prix circuit runs anti-clockwise, these tyres are subjected to greater stress.

    Shortly before the two-hour mark, Andlauer came under pressure from the two Cadillac of Team Jota, who were able to take the lead despite his fierce resistance. This remained the case in the middle phase of the fifth race of the season, when Michael Christensen from Denmark took the wheel. It was only in the final phase, when the already low tarmac temperatures dropped below 25 degrees Celsius, that the hybrid prototype of the two former Porsche Juniors came more strongly to the fore again. An exciting chase broke out in the final hour of the race, in which Andlauer was able to reduce a gap of a good eight seconds to less than a second at times – but an attack on second place was not to happen.

    Laurens Vanthoor started the race in the number 6 sister car. The Belgian initially lost one position, but quickly made up two places. After the changeover to Kévin Estre from France, the 501 kW (681 PS) race car from Weissach initially continued to run in sixth position. At the halfway point of the race, the two reigning FIA World Endurance Champions were in fourth place and defended this result to the finish.

    “Third and fourth place is a very good result for us in terms of the championship,” emphasised Urs Kuratle, Director Factory Motorsport LMDh. “We weren’t able to beat Cadillac today, but we had everyone else in our pocket. Our Porsche 963 worked perfectly, and the team and drivers also did a flawless job over the entire weekend. That’s going in the right direction, now we’re looking forward to the rest of the season.”

    “Our number 5 Porsche executed our race strategy really well and we are very proud of the podium result. The sister car came from a little further back and was able to realise fourth place, which we are also happy with,” explains Travis Law, Technical Director Porsche Penske Motorsport.

    The Porsche customer team Proton Competition was delighted with its best result of the season so far in Sao Paulo with tenth place. The German team is fielding the third 963 in the FIA WEC. As the other hypercars had not previously tested on the newly paved Grand Prix circuit either, Nico Varrone from Argentina, Neel Jani from Switzerland and Nico Pino from Chile started the race with a comparable wealth of experience.

    LMGT3 class: best race result of the season for Iron Dames

    Fourth place in the LMGT3 category for Célia Martin from France, Rahel Frey from Switzerland and Michelle Gatting from Denmark: the Iron Dames celebrated their breakthrough in Brazil and achieved their best WEC result with the 911 GT3 R. The successful initiative to promote girls and women in motorsport is cooperating closely with Porsche Motorsport from this year onwards. The day before, the trio had already made it into the Hyperpole session of the ten best qualifiers for the first time. In the race, all three impressed with a strong driving performance and demonstrated their fighting spirit in numerous duels for position. Their number 85 car set the fastest lap times over long stretches. Until three minutes before the end of the six-hour distance, it even looked like a podium result was in the cards. However, Gatting then had to let an Aston Martin pass.

    Thanks to another flawless race, the 911 GT3 R of Manthey 1st Phorm was able to work its way up from 13th on the grid to sixth position. The American Ryan Hardwick, Riccardo Pera from Italy and the Austrian Porsche factory driver Richard Lietz had previously won their class at the 24 Hours of Le Mans and had to pack on 36 kilograms as a success ballast in Sao Paulo. Nevertheless, the trio extended their lead in the LMGT3 drivers’ standings with this result.

    Drivers’ comments after the race

    Michael Christensen (Porsche 963 #5):
    “Third place is a good result for us. We did our best, but the Cadillacs were simply faster than us today. Julien got off to a great start and took the lead. However, we were unable to defend first place over the distance. We can be satisfied with our performance, even though our expectations are obviously different. Looking at the season so far, the development is pointing in the right direction: we are getting stronger and stronger.”

    Laurens Vanthoor (Porsche 963 #6): “Fourth place is probably the best we could achieve today. The starting phase was very difficult because we first had to fight our way through the traffic. After that, there was already quite a gap to the front that we couldn’t close, because the Cadillac was simply the fastest car today.”

    Nico Pino (Porsche 963 #99): “With tenth place, we were finally able to finish in the points! We’ve learnt a lot in the last few races and made good progress. We now have a better understanding of how we need to set up the Porsche 963. Now we are even more optimistic about the future. We can still improve in one or two details.”

    Rahel Frey (Porsche 911 GT3 R #85): “Fourth place feels bittersweet – on the one hand, the Iron Dames have achieved their best WEC result to date. On the other hand, we were on the podium until just before the chequered flag. A big compliment goes to the Manthey team for the perfect preparation of our Porsche 911 GT3 R. It’s incredible fun to drive this race car. All three of us had great duels on the track.”

    Riccardo Pera (Porsche 911 GT3 R #92): “Because of our success ballast, I would never have thought our result today was possible, because the 36 extra kilograms really slowed us down. Against this background, sixth place is simply outstanding – especially as our direct rivals didn’t score any points. As a result, we were able to extend our lead in the overall LMGT3 standings.”

    The Sharks Are Circling OpenAI

    Despite all the competition, OpenAI is still the leader in the space and is making its own moves that will likely worry rivals.

    It is planning to launch a new AI-powered web browser, for instance, that could compete with Google Chrome, the current industry leader. The browser will embed ChatGPT and feature an AI agent that can handle tasks like booking reservations and filling out forms.

    It also secured a $200 million contract to provide AI support to the US military. OpenAI will help develop capabilities to “address critical national security challenges in both warfighting and enterprise domains,” the Pentagon said in June. OpenAI earlier partnered with Palmer Luckey’s defense tech firm, Anduril.

    OpenAI is also forming more playful partnerships. Last month, Mattel announced it was working with OpenAI to bring AI to its iconic doll, Barbie.

    By using OpenAI’s technology, Mattel will “bring the magic of AI to age-appropriate play experiences with an emphasis on innovation, privacy, and safety,” the California-based toy manufacturer said in a press.

    Altman, for his part, is at least publicly optimistic.

    “I have never seen growth in any company, one that I’ve been involved with or not, like this,” Altman said at a TED conference in Vancouver in April. “The growth of ChatGPT — it is really fun. I feel deeply honored. But it is crazy to live through.”

    James Gunn on Why He Got Bradley Cooper For Important ‘Superman’ Cameo

    SPOILER ALERT: This article contains minor spoilers for “Superman,” in theaters now.

    James Gunn has high praise for Bradley Cooper, which is why he needed him for a very important cameo in “Superman.”

    In the opening moments of the DC tentpole, Cooper appears as Jor-El, the father of Superman. The role was first brought to the big screen by legendary actor Marlon Brando in the original 1978 “Superman.” During a recent interview with Entertainment Tonight, Gunn said he cast Cooper with Brando’s performance in mind, knowing he could keep with the foremost portrayal of Jor-El.

    “I needed somebody who could play Jor-El, who had the stature of what we imagine that character to be,” Gunn explained. “Somebody that could walk in the footsteps of Marlon Brando.”

    Gunn previously collaborated with Cooper on the “Guardians of the Galaxy” trilogy, in which the “Silver Linings Playbook” star played Rocket Raccoon. The director said on an episode of “Jake’s Takes” that the relationship he formed with Cooper during those films made it easy to lock him in for “Superman.”

    “He’s a friend,” Gunn said. “We’ve stayed in close contact since the ‘Guardians’ movies, and I admire him greatly as an actor and as a director.” Gunn added that he called Cooper and asked, “Hey, will you do me a favor? Come down, go to England, we’re going to shoot you in a 3D environment, make a hologram of you, and you can play Jor-El.”

    ROG Xbox Ally and ROG Xbox Ally X price leak at Asus store may not quiet critics of Xbox handhelds

    Despite a rumored October release date, Microsoft hasn’t announced the ROG Xbox Ally or ROG Xbox Ally X price. According to an Asus partner’s website, the Xbox handhelds will cost €599/€899. The leaked information comes after insiders question the appeal of the systems.

    After leaker eXtas1s revealed the price of the new Xbox handheld options in June, there is evidence to support his claims. The Asus Store by MacMan is an official Spanish partner of the handheld gaming PC manufacturer. When searching for “Xbox Ally” in the site’s search bar, pricing appears. If the store is correct, the ROG Xbox Ally X price will be €599, while the ROG Xbox Ally is a steeper €899.

    Pricing for the U.S. market is less certain, although eXtas1s suggested $499 and $799 as educated guesses. Other analysts have predicted that the systems will be slightly more expensive, with tariff uncertainty a factor. While Microsoft announced a Holiday 2025 release date for the ROG Xbox Ally series, Windows Central’s Jez Corden thinks October is possible.

    Microsoft officially unveiled the machines at its June 8th Xbox Games Showcase. Rather than creating a new console, the Xbox handheld offerings are similar to the existing Asus ROG Ally lineup. Gamers can expect the same 7-inch 1080p/120Hz displays, as Asus has decided against using costlier OLED panels. However, besides an Xbox control scheme, Microsoft promises grips with improved ergonomics.

    There is plenty of skepticism among gamers and insiders about the new systems. Former Microsoft Executive Laura Fryer views the handhelds as a way for the company to abandon hardware. Fryer notes that ROG Xbox Ally candidates lack the simplicity of a traditional console. Also, after shifting to a cross-platform approach, there will be few exclusive Xbox games to tempt buyers.

    To entice a wider audience, Microsoft will introduce a full-screen interface with access to multiple marketplaces. Nevertheless, the ROG Xbox Ally and ROG Xbox Ally X prices may decide their fate. The likely cheaper Nintendo Switch 2, along with its support for more AAA titles, tempts console gamers. Meanwhile, a competitive handheld gaming PC landscape includes the Steam Deck and multiple Windows-based alternatives.
